URGENT H1N1 ALERT INFORMATION
LAST CLINIC IN 2009*
Alert Message-H1N1 Community Vaccination Clinic
WHEN: Wednesday, December 16, 3-5pm
WHERE: Town Building
First Come-First Serve
• For all children 6 months to 18 years
• For healthy adults to age 49 without underlying health issues
• For adults 18-64 with documented health issue.**
* Additional clinics will follow in January 2010.
** Proof of high-risk medical conditions (inhaler for asthma, prescription, for example) for high risk categories will be required.
H1N1 Flu Clinic Saturday December 12, 10am to 2pm, Town Building
- All children ages 6mos-18. Children with underlying health issues that will require injectable vaccine will need to provide recent prescription for asthma medication or letter from doctor verifying health issues and need for injectable vaccine.
- There are also 500 mist doses for healthy adults to and including age 49 (with no underlying health issues)

Sudbury-Wayland-Lincoln Domestic Violence Roundtable is an all-volunteer nonprofit organization of men and women incorporated in 1999. The goal of the Roundtable is to raise awareness about the issue of domestic violence through community education and networking and to improve the coordination between public and private services for victims and families touched by domestic violence.
